Patella instability is a relatively frequent event in young patients. Both conservative and surgical treatment may be proposed. Several procedures have been proposed in the past decades reflecting the complexity of this pathology. Recently the role of medial retinaculum and its components as a passive and active medial restraint have been investigated. Thus open and arthroscopic medial reefing have been proposed in case of failed conservative treatment in patients with patella instability or painful unstable patella. However in our experience patellar dislocation represent a counter indication to arthroscopic medial reefing. We describe our surgical technique as well as postoperative rehabilitation protocol. Clinical outcomes confirm that medial reefing guarantees good subjective and objective results if correct indications are given.
