In order to ensure maximum safety in the provision of health services, especially in the case of small patients, it is also necessary to know the risks and the possibilities of its reduction and elimination in the field of transfusion medicine. Patient Blood Management (PBM) is indicated internationally as the standard of care in the transfusion field. The implementation of this multimodal program has shown that a blood transfusion is not always necessary or inevitable but can be considered modifiable. The literature review confirms that transfusion should not be the default option for managing anemia or blood loss. On the contrary, a proactive approach centered on the patient’s blood is able to guarantee better clinical outcomes and lower risks for patients. In the pediatric field, knowledge regarding this approach important and should be taken into consideration in order to provide the best possible care to small patients who for various reasons may be anemic.
