Background: The ratio of two metabolites derived from nicotinesmoking, 3’-hydroxycotinine (3HC) and cotinine (COT) (nicotinemetabolite ratio, NMR), reflects the activity of the liver enzymeCYP2A6 and it has been shown to predict response to smoking cessationdrugs in clinical trials. The aim of the present study is tofacilitate translation of study findings in the real life clinical practice. Methods: We carried out a clinical validation of a mass spectrometerbasedmethod for measuring the NMR in both plasma and salivasamples obtained from 71 smokers attending the Smoking CessationCentre of the University Hospital of Pisa. We also performed a realtimePCR analysis for fast genotyping of CYP2A6.
